Documentation:
The following documentation is required when taking out a claim:
• Fully completed claim form.
• Certified copy of ID document of the deceased.
• Certified copy of ID document of the assured person.
• Copy of the BI 1663.
• Police report for all accidental deaths.
• Child: birth certificate or Road to Health Chart should aforementioned not be available.
• Latest medical report for all mentally-handicapped children.
• Confirmation of all students from educational institution with which they are registered.
• Commissioner of oaths to certify all copies.
Please note the following:
• Claims need to be deported within 6 months after death has occurred.
• Insurer reserves the right to request additional information that is required in the claim process.
• Upon receipt of all documentation, the claim is to be submitted to the Dignity Claims Department for approval.
• Dignity can approve or repudiate claims on behalf of the underwriter.
• Once claim is approved, the amount covered will be deposited into the banking account provided by Dignity.
